IntroductionThe criminal justice system of Texas (U.S.A) covers both criminal justice and civil justice, consist of law enforcement, law adjudication (the court system), and criminal corrections. Law enforcers includes; attorneys, sheriff and police departments, and the state Department of Public Safety. Whilst, criminal corrections includes; prisons, pardons, and paroles.Additionally, the criminal justice in Texas involves Office of the Attorney General (OAG). The Office of the Attorney General is the state’s top lawyer and law enforcer, which is elected by the citizens of Texas. The prime responsibility of the Office of the Attorney General includes; defending the laws and constitution of the state, represent the state in lawsuit / court cases, and commend on bond issues.In addition, the responsibilities of the Office of the Attorney General includes; â€Å"heading of state agencies and other state officials and agencies, defending against challenges to state laws and suits against both state agencies and individual employees of the state, issuing legal opinions when requested by the Governor, and serving as legal counsel to all boards and agencies of state government†.The Antitrust and Civil Medicaid Fraud Division and Consumer Protection Division investigate in violations of state and federal antitrust statues, and consumer fraud and civil lawsuits. Both    bureaus of the state are responsible in prosecuting such cases.Types of LawThe justice system of Texas are divided into; Civil law, Criminal law and Administrative law. The civil law covers ant disputes between; individuals and / corporations in the state. Criminal law encompasses crimes committed against the state, and any violations under the laws of the state.Administrative laws include laws set and enforces by the government other than the legislative body of the state. For example, the state sets curfew among minors and imposes fines to violators. To be able to put on trial the alleg ed citizen, the government of the state does have local courts, district (law) firms, lawyers, forensic laboratories, police and jails.Criminal Justice SystemIn general, most citizens wanted to punished criminals harshly, treat them as they wouldn’t create any crime in the future. In most of the states, the justice system provides a moral trade-off between public safety and the protection of the rights of the criminals. Texas criminal justice system prefers immediate and strict punishment for the citizens violating the laws of the state.The criminal justice system in Texas has three components; law enforcement and criminal prosecution, criminal trial and appeals, corrections. Each component of the criminal justice system of Texas comprises hundreds of employees. The Department of Public Safety as part of the criminal justice system of the state patrols roadways and other facilities of the state.The court system of Texas follows a hierarchy, in which, local courts are decentra lized among municipalities of Texas. The Supreme Court of Texas functions to regulate policies and laws among local courts. Unlike in other countries, the states in America do have each supreme courts and the county (state) are considered autonomous / taken apart from the country’s policies and laws.Each of the components of the criminal justice of the state does function separately, with its own structure and implementation of its duties. Even though the criminal justice system of the state set standards on how the crimes will be investigated and prosecuted, this varies among localities / municipalities in the state. On the other hand, the criminal justice system in Texas performs strict policies despite its decentralization of the system. Most of the criminals are prosecuted and put up into jail (Brauchli, 2005).Sentencing and PenaltiesSentencing among criminals is based on the graded penalties which specify the degree of penalties to be imposed with regards to the degree o f crime the accused done. The degree of crime is differentiated between misdemeanor and felonies. Misdemeanors are crimes with lower degree of punishment compare to felonies. Misdemeanor does involve petty crimes such as, minor drug possessions, generally crimes without involvement in injuring a person / citizen of the state. On the other hand, felonies are more serious crimes than misdemeanors. This includes; theft, homicide, and damages of state’s property. Both cases are prosecuted in district courts, severe crimes are prosecuted at the Supreme Court of Texas (The Justice System, 2006).Misdemeanor are considered minor crimes with minor punishments (graded in classes) while felonies are penalized by degrees. Upon committing felonial crime, the criminals are convicted to death. Repeat offenders; involve assigning higher degree of felony. The criminal in return, may be convicted to a higher degree of punishment and in worst scenario, amputation of human rights of the accused. Implications of Policies to Criminal Justice System of TexasThe society set standards to what their state would behave and the policies to be implemented. The policies and laws imposed to provide the criminal justice system of the state a guide on how the system will investigate and prosecute crimes, accused the criminals, and punish them accordingly to the degree of the crime they committed. The criminal justice of the state is transparent, meaning that the system works to defend the constitution, protect the citizens of Texas against criminals, and provide the citizens policies in order to maintain peace in the state.The criminal justice system of the state is not being harsh and cruel to its citizens; the system only maintains upright decisions on which the citizens can expect fair treatment among any individuals. Weak implementation of policies and laws confers unfair justice, abuse of public property and upheaval.Wherever the citizens vote for the Sheriff and / or the Attorney General, the state shows the importance of democracy in Texas. The fast punishment to criminals made the justice system of the state respectable and fair. Criminals are subjected to prisonment if the court found the accused guilty. In short terms, the policies and laws are applied and not misused.On other hand, the criminal justice system of the state creates a back lag because of many positions allotted in the system. The system creates inefficiency in terms of uses of public funds, and ineffectivity in running the office (Texas, 2006).

Personal Development Plan as A Leader Essay

1.0 IntroductionThis report concerns the general framework of my professional and personal leadership development. The details I will be presenting are what I consider to be in my current or future role for my clients, my team, my organization and my self. It is followed by the narratives of what I look like at my best with the understanding of my previous experiences. Then employ literature to review why those two tasks are important for leadership development. This report will reflect how I perform in current role with others and also the skills and strengths I have developed and discovered. 2.0 An effective organizational leaderThe success of an organization depends on people working together and sharing a common purpose. The leaders need to focus on the workforce to identify their individual human needs. Leaders are affected by the constantly changing environment in which globalization plays a key role. Accordingly, the workforce is in the trend of diversification. Hence, an effective leader is an individual who displays transactional and transformational leadership. To be an effective organizational leader, we must know the dimensions of two leadership styles. In a study by Zhu, Chew and Spangler (2005) and Daft (1999), a transactional leader is an individual who clarifies subordinates role and task requirements, initiates structure, provides rewards, and displays consideration for subordinates. To meet these requirements within the business, the leader must be able to adjust the style of leadership to satisfy the subordinates. When subordinates demonstrate a low readiness level to achieve tasks, leaders need to adopt a directive style of leading that gives employees explicit directions on how tasks should be accomplished. However, when the readiness level increases, leaders should be able to adjust to a more delegating style of leadership that gives subordinates the responsibility for making and implementing decisions. A transformational leadership goes beyond transactional leadership techniques. Tickle, Brownlee and Nailon (2005) state that a transformational leader has the ability to inspire and motivate people to do more than the  call of duty, in addition, innovative by bringing changes within the business such as upsizing the business entity. Daft (1999) claims a transformational leader has the ability to get people to embrace a common purpose and to implement a visionary idea that will excite, stimulate, and drive other people to work hard. This involves the leader being able to listen, in order to learn what moves people. Leaders who motivate are able to communicate in a way that inspires people. There is often an aesthetic element to their visions. They not only communicate information, but also a sense of meaning that inspires people to follow, even if it may lead to sacrifice in terms of hard work, long hours and deferred rewards. Anyone can be a leader, but a successful leader is an individual whose behavior more reflects the transformational leadership style with the use of personal power to influence people to achieve the goals of the business. The left column of the table above is the four areas that I will focus on to develop my leadership skills. For each area, there are several criteria to assess my current personal skills. The items listed in the â€Å"I need to be† column are my goals and criteria for monitoring process of my leadership development. In the box of â€Å"for my clients†, I have put higher scores for my current role because from my previous education consultant experiences, clients are very important for business sustain. Therefore, in order to maintain an on-going maturing relationship with clients, I need to treat them with courtesy, respect and responsive, for difficult clients I need to be tactful. In the area of â€Å"for my team†, I assessed myself at a lower medium level for my current role. I don’t have much group work experience from previous job. Therefore, this will be the most important area where I need to develop, because in current business organizations, team works and synergy are vital for completing multiple tasks. It is very important to learn how to convey the key lessons of organizational effectiveness through the stories of my own workplace experiences. And I currently lack of this skill. In a study by Tichy (2002), this is known as the narrative teachable point of view, which is the best communicated through engaging stories about who the company is and about what it can be. In the area of â€Å"for my organization†, I regard â€Å"reliable† as the primary element, because to be an effective leader of organization, I need to be reliable to obtain trusts from the company and team members in order to perform tasks effectively and work with others by sharing the same objectives. Also, creative strategies and innovation skills are very important for company growth and expansion. The area of â€Å"for my self† is designed to develop my personal leadership skills. The listed items are essentials to enhance my effectiveness. According to the evaluation, my assertiveness and courage are poor because of the lack of communication skills. I need to enhance my communication competency to be a confident professional leader. Other aspects such as optimism, calmness and buoyancy are the secondary area where I need to improve because those attributes are important to demonstrate a good personal leadership. All the elements listed in the table are essential for developing a transformational leader and keeping the process on the right track. From the table above I will be clear about my weaknesses and resolve them. 4.0 Gaps between my worst self and the idealI think that lifelong learning is very important and we all have personal worst self and ideal self. Knowing my own strengths and weaknesses can be helpful for me to become an effective transformational leader. Discovering my worst self along with having the yearning to improve is a talent that I maintain. Everybody has exceptional traits which distinguish the individual. Hence, I need to identify my ideal self in order to overcome my worst self. I believe that my ideal self has always made me be successful in projects I have attempted in my life. Therefore, recognizing ideal and worst self is a key point to bring success. For instance, when I come across a limitation in an area of necessity, I am able to improve that weak point. I deem that this characteristic is a strength that I have. I implement my personal strengths in my job. They are problem solving, goal oriented and so on. I feel that these strengths have helped me go ahead in my career. For instance, it is easy for me to analyze a problem and come up with an effective resolution in a short period of time. Focusing on the whole outlook but not the problem itself enabled me to resolve many problems in the past. It seems that if one narrows the situation into smaller workable areas, the problem is solved more quickly. I believe that the work that I produce is an indication of what kind of person I am. I like to make high values for my occupation and myself. I have found that my employer has always been satisfied with my work. For instance, I am able to work with colleagues to obtain the best outcomes in projects and problems solving in the workplace. I consider myself as a goal-oriented person. I enjoy setting goals for myself and I like to work with my colleagues in setting these goals. I feel that  once a goal has been achieved, we can rejoice our accomplishment as a team. For example, we have twenty students’ visas approved from immigration and we had a cocktail party to celebrate. One area of worst self is that it is difficult for me to be an effective leader. Because I feel that it’s very hard to lead a team efficiently and I had some difficulties to work with unfamiliar people. My overall goal is that I can be an effective transformational leader. I know how to learn, along with the understanding of how to learn the best, which is an important part in learning the goals that I have set for myself. As a student of RMIT University, from this course I learned how to improve my effective leadership skills so that I can improve that weakness. This is my second semester at RMIT University and I will overcome the new challenges that I will face. I hope that the experience I will gain while obtaining my degree will continue to strengthen all my personal and professional skills as well as challenging me in the areas of weaknesses. 5.0 My best self narrativeMy best self narrative was when I overcome the nervous and dilemma and encourage myself to finish the tasks. I had a lot of presentations in this MBA course and still remembered the scene of my first presentation. I was standing in front of all colleagues and lecture. My hands started dithering and I could not remember what I prepared for the topic. Luckily, I had my keynotes. I started the PowerPoint slides, reading the topic and facing to all audiences. Suddenly, all the information was back to my mind and I talked to myself that I can do this. Finally the presentation was really good and I gained an upper distinction grade from this assignment. Another best-self narrative is when I gained recognitions from my family and relatives. I have been studying overseas for 6 years and still remembered the first time when I returned to my home country. All my relatives went to my parents’ house and praised my abilities and talent for completing my IT bachelor degree and easily gaining the permanent resident visa. Because they  thought that it was very hard to finish all the tasks by myself while I am very far away from family and without any support. From that moment, I felt that I had done something valuable, gaining recognitions from my family and relatives. I believe that the successes of my best-self experiences were from the supports and encouragements of my family and friends, which constitute me a â€Å"can do† attitude. The recognition obtained from relatives gives me more confidence and ambitious dreams about what I want to do in my future. Also, my personal experiences of failing make me learn how to face frustration. Based on these experiences, I gained courage and fortitude for further problem resolution and kept my self at the best. 6.0 Best reflected self exerciseI have asked about 10 people from my friends and colleagues to conduct a best reflected self exercise and discover my strengths from their point of view. With the aspiration and analytical abilities, I perform tasks much easier. Friends think that I am willing to do the best with empathy, which brings my reliabilities and performing attitude to friends. One of my best strengths is willingness to provide a helping hand whenever I can; I believe that everyone does need help from others including my self. Therefore, helping others will gain help from others. Colleagues think that I am able to take care of others and act as an older brother, which can show that I am a potential leader with a set of leadership skills when I am at my best. And they believe that I am able to identify the goals of tasks and allocate those tasks to group members rationally. So they feel more confident to work with me. Also, they mentioned that my leadership style is towards to transformational leadership that leaders encourage and motivate members by sharing the same goal and achieve the success. I believe that those strengths mentioned above will enhance my understanding of the work situations that can bring out the best from me. Also, by acknowledging those strengths, I will be more confident to do my tasks and  work with other people. 7.0 Leadership narrative This section will discuss what event makes me realized that I want to be a leader and shows the shift to leadership. I remembered when I worked as an education agent for a consultant company. The company has 7 employees including myself. Ken, the manager of the company, normally have a 7-week business trip to China every 8 months. Since I was the oldest employee in this firm and had been working in the company for over 3 years, he talked to me privately and asked me to manage the company and make sure everyone started working on time. The first day after Ken left, some of the employees were very happy, starting coming to work a little bit late and taking a longer morning tea break. Gradually, they were getting even worse, the deadline of tasks delayed and they disappeared one hour early before company closed. I talked to them, and asked them to complete the tasks and come to work on time, but they told me that you were actually not in charge here and we were on the same level. Then I explained what Ken told me to manage the company and monitor the attendance. Also, I displayed my self as an example to work hard on tasks and keep coming to work on time. Eventually, they started turning back to the normal attitude as before, and we had a good time to collaborate with each other. When Ken came back, the performance was increased and he appreciated my excellent job while he was away. The work mission and my manager’s trust are the key elements to drive me to get the opportunity to be a substitute leader and solve problems occurred in the company. According to this experience, I found that I had the potential skills and abilities to lead a group. From that moment, I started thinking about my future role in the company. I wish to be a good business leader. 8.0 Literature Review The two tasks, completing how I need to be in my current (or future) role and re-authoring my understanding of my own key experiences at my best, help me to evaluate myself. And also I could see my self from others’ view. Most importantly, those two tasks inform me where I am right  now, who I want to be and the approaches that I can take to step forward. The literature review has been conducted in this section to explain why undertaking those two tasks increase the likelihood of experiencing the buoyant, positive emotions that support the effective leadership. Tickle, Brownlee and Nailon (2005) believe that underlying and enabling transformational behaviors, is a set of core beliefs about knowing and learning which can be called â€Å"epistemological beliefs† means what knowledge is and how learning and development occurs. Tichy (2002) supports their point of view based on his experience with many outstanding business leaders at GE and states every successful business leader must have a teachable point of view, the ideas they have to improve the business, the values that will make those ideas work and the ability to energize employees. In addition, Tichy (2002) mentioned the leader’s teachable point of view is best communicated through engaging stories of who the company is and of what it can be. Jack Welch is a good example to illustrate this point, he created a story for the future of the GE based on the elements of his own ‘who I am’ story, and employees shared the view from him and execute base on this view. In a study by Morgan et al (2005), they define four steps to take in order to go through RBS exercise. Those steps are identifying respondents and ask for feedback, recognize patterns, compose your self-portrait and redesign your job. To have a better understanding of RBS exercise, Morgan et al (2005) suggest the first step is to collect feedback from a variety of people inside and outside work. The second step is to search for common themes among the feedbacks. The third step is to write a description of individual that summarizes and distills the accumulated information. The final step is to redesign personal job description to build on what is good at. Also, Morgan et al. (2005) state â€Å"the RBS exercise can help you tap into unrecognized and unexplored areas of potential. It armed with a constructive, systematic process for gathering and analyzing data about your best self, you can burnish your performance at work†. Wright (1996) indicates transformational leadership involves a highly positive emotional and optimistic thinking response towards the leader. The values of those two attributes are very important for effective transformational leader. The optimistic thinking requires turning negative thoughts into positive, which will influence other employees, feel more comfortable and confident to work for the organization. Also it helps individuals to have a great level of energy with reduced stress and tension in organization when approach to any tasks. The positive emotions provide the confidence to leaders and reflect on the best self evaluation which is what you are like when you are at your best, similar to an â€Å"ideal self†. It focuses on increasing the advantage instead of concerning to improve the disadvantages. An article written by Botaris (2003) discusses emerging key trends, such as leadership development of effective judgment. Effective judgment in the future will continue to change because people will change. Leadership effective judgment will have to become more strategic, more proactive, and more involved in the overall business (Bates, 2002). Transformational leadership style will be the best suited to those trends. Leadership development of effective judgment is a crucial function of management in order to facilitate the vision required for an organization to be successful. Effective judgment is the process of reducing employees’ resistance to change. Management directs employees’ efforts towards the accomplishment of the organizational goals and objectives. Ibrarra and lineback (2005, p 66) states that â€Å"To know someone well is to know their story†. A successful leader needs ability to go on to point out that at times of change in the professional direction. The leader needs tell the story that makes sense of personal motives, character and the capacity to reach new goals. In other words, this is called narrative abilities, the narrative will make an enormous difference in how well the company will cope with change, how high is the level of motivation of employees, how devoted employees to organization. Having effective narrative style helps leaders become more assertive and influence in convincing other people to do the tasks. It is important to understand that â€Å"creating a story that resonates  also helps up to believe in ourselves† (Ibrarra & Lineback, 2005, p.66). To make a smooth construction of narrative, Gibson (2004) discusses some examples and analysis for understanding who you have been and for identify ing where you might need to go next. He argues that taking a narrative approach to career counseling is both theoretically and practically justified. In a study by Cervone, Shadel, Smith, and Fiori (2006), they propose a model of intra-individual personality architecture called as KAPA: A Knowledge and Appraisal Personality Architecture. This model helps individuals to appraise themselves and situations based upon beliefs that they have come to experience as knowledge. The KAPA model will guide individuals to set goals and improve them. The KAPA model has three conceptual principles, notion of intentionality, distinguishes between two aspects of cognition and differentiates among alternative forms of knowledge and appraisal. I have used the KAPA model to appraise myself to test my personalities, skills and beliefs, please refer to Appendix 1 for my KAPA analysis. In conclusion, I have identified my current (or future) role and evaluated myself at this stage. And my own experiences in the narrative stories show the constitution of my identity and the turning point to be a business leader. Then, the literature review explains the benefit to do those two tasks and importance to support effective leadership. From this paper, I have found out my strengths from others’ point of view and I have identified the gap between my best and worst self. A Literature Review About Mecication Errors Essay ? An error rate of 5% is acceptable in most industries, however, in the health care industry; one single error can result in death. (Berntsen, 2004, p5) This paper discusses medication errors in relation to pharmacology and drug treatment. It will summarize three academic peer reviewed journal articles, followed by general information in relation to medication errors, the impact of medication errors on client care, strategies to prevent medication errors  and conclude with the relationship to nursing. Summary of Articles Related to Medication Errors. The first article is by Karin Berntsen, 2004, and is entitled â€Å"How Far Has Health Care Come Since ‘To Err is Human’? Exploring Use of Medical Error Data†. This is a review of what changes have been made since a medication error report written by the Institute of Medicine was published in 1999. This article depicts how the health care system has changed since this 1999 report was written, and how the information was utilized for our benefit. They concluded that in the USA, medical errors were one of the top 8 leading causes of death. They reported the cost for these errors was between $17 Billion to $29 billion dollars. Until a new report is completed, health care providers will be unaware whether their goals in increasing patient safety were accomplished. The article finalizes that there has been progress in regards to prevention of medication errors and health care leaders feel passionate about increasing patient safety. (Berntsen 2004) The second article is by William N. Kelly, 2004, and is titled â€Å"Medication Errors: Lessons Learned and Actions Needed† and highlights the death of a one year old child who was diagnosed with cancer. She subsequently died, not from the cancer, but from receiving an incorrect dosage of a drug that she was being treated with. This report indicates that medications are systematically checked and balanced and errors are usually caught before a drug is administered to a patient. The article states that problems are not being solved in a timely manner since the industry has been â€Å"putting ‘band aids’ on problems that need ‘major surgery’.(Kelly 2004). In conclusion, the article questions whether or not they are taking the right approach in preventing errors. Many people are trying to fix this problem however; errors are still made too frequently. (Kelly 2004) The final article is by Rosemary M. Preston, 2004, and is titled â€Å"Drug Errors and Patient Safety: A Need for Change in Practice†. This article presents that errors continue to happen for many reasons. It concentrates upon calculations errors, lack of knowledge of drugs, over/under dosing drugs,  interactions with drugs and food, and legalities regarding drug administration. It also presents recommendations to minimize the risk of drug errors with good communication and honesty. The article closes by stating that â€Å"nurses should never estimate the skills needed for safe administration of medicines.† (Preston 2004) Key aspects: medication errors and their causes. To understand the impact that medication errors have on a patient, we have to understand what a medication error is. According to Health Canada online, a medication error is defined as: Any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is in the control of the health care professional, patient, or consumer. Such events may be related to professional practice, health care products, procedures, and systems, including prescribing; order communication; product labelling, packaging, and nomenclature; compounding; dispensing; distribution; administration; education; monitoring; and use.† [Developed for use by the National Coordinating Council on Medication Error Reporting and Prevention]( http://www.hc-sc.gc.ca/english/index.html) Medication errors occur for a variety of reasons. An error can affect all areas of a health care facility from health care management, staff, physicians, pharmacy and especially patients. Studies have indicated that errors will usually occur when the staff demonstrates signs of fatigue, stress, are over-worked or encounter frequent interruptions and distractions. When physicians display bad handwriting, ineffective communication with patients, and do not educate staff and patients effectively, a medication error is more likely to happen. Poor management can result in more medication errors when there is an emphasis on volume, over service quality. This results in inadequate staffing and disorganization. Medication errors affect all components of the health care environment. (http://www.napra.org/docs/0/95/157-/166.asp) As disturbing as it sounds, one miniscule error can result in a patient’s injury or can even lead to their death. According to the American Journal of Medicine, statistics reveal that â€Å"more than two million American hospitalized patients suffered a serious adverse drug reaction in relation to injury within the 12-month period and, of these, over 100,000 died as a result.† http://www4.nationalacademies.org/news) Death and injury is a sad reality to any single error. The government established six rights of drug administration to prevent medication errors and ensure accuracy. These six rights include: Right drug, right dose, Right client, right route, right time and right documentation. (Kozier & Erb 2004) Injuries that result from a medication error are called adverse drug events. Usually, these unpleasant effects can be eliminated and injury can be avoided. However, every drug produces harmful side effects, but the severities of these effects vary from individual to individual. These side effects also depend on the drug and the dose given. (Kozier & Erb 2004) Health care professionals must report all errors and are accountable for their actions. No matter how insignificant, nurses are taught to document and report all mistakes. When statistics show what types of errors are made, an analysis can be done. This analysis can be used to plan ways to prevent them medication errors. (Berntsen, 2004)When a nurse does not report a mistake, the probability that it will happen again will increase. Medication errors have a huge impact on client care. They can result in death, injury, and result in unwanted effects of drugs. It is our responsibility as nurses to comply with the clients’ six rights of drug administration, to prevent errors from taking place. Strategies to prevent medication errors. There are many efficient ways to prevent nurses from making an error. To ensure patient safety in all aspects of client care, nurses are taught to think critically, and to problem solve. Nurses use critical thinking to ensure safe, knowledgeable, nursing performance and they must be able to keep up with updated health facts by constantly educating themselves with new information. (Kozier & Erb 2004) Critical thinking assists in the prevention of medication errors. The six rights in drug administration help prevent medication errors from occurring. It is important to maintain the highest standards of practice of these rights for a drug to be prepared properly. Failure to adhere to any one of these rights will definitely result in a medication error. (Clayton & Stock, 2004) Take your time when preparing medications and research any unknown drugs. Rushing should be avoided when preparing, administering and reading medication labels. Proper research must be done before an unfamiliar drug is administered it to a client. Even when in a rushed emergency situation, drugs should be looked at carefully to know the correct concentration and name of the drug, to prevent injury. (http://www.hc-sc.gc.ca/english/index.html) Labels should be read carefully and accurately. Before a drug is given to a patient, three checks should be done to ensure you are giving the proper drug and dose. In a situation where you are unsure of a drug order, you are expected to refuse the order and clarify it by law. If an individual is unfamiliar with a particular drug, the drug should not be given. (http://www.napra.org/docs/0/95/157-/166.asp) When a label is unclear, do not try not to examine the drug order yourself. Do not ask an associate, or ask for anyone else’s interpretation of the drug. To get the correct information, contact the individual who ordered the drug to clarify the label. In order to decrease the chances of error, verify all unclear hand writing, abbreviations, decimal points, decimal places and dosages. (http://www.napra.org/docs/0/95/157-/166.asp) Use of dosage abbreviations should not be used to avoid drug miscalculations. Dosage abbreviations are misinterpreted more often, than any other type of abbreviation. Using standardized abbreviations, would assist in preventing misinterpretation of abbreviations. (Preston 2004) A drug check should be done three times prior to the administration of a drug. The drug label should correspond with the physician’s orders. The three checks should be done; â€Å"Before removing the drug from the shelf or dosage cart, before preparing or measuring the actual prescribed dose, and before replacing the drug on the shelf or before opening a unit dose container, just before administering a dose to a patient.† (Clayton & Stock, 2004) Do not make assumptions regarding drugs. Physicians, pharmacists, make mistakes and other parts of the health system may be flawed. For example, when documentation shows the patient has no drug allergy, it is wrongful to assume the patient will have no adverse reaction to a new drug. This could result in detrimental results to a client’s health. Therefore no assumptions should ever me made. (http://www.ismp.org-/ToolsAllina-Orientation.html) A quiet environment for preparing medications will prevent prescription errors from occurring. Sometimes, nurses are repeatedly interrupted when preparing a medication. Distractions interfere with processing information and decision making. Errors will least likely occur when preparations are done when there are no distractions. (http://www.ismp.org-/ToolsAllina-Orientation.html) When preventing errors, staff must be certain all dosage calculations are correct and clarified. It may be beneficial to ask a colleague to assist you in checking doses, to minimize the chance of miscalculations. Other suggestions to minimize error include; â€Å"making pre-calculated conversion cards, always use a leading zero before a decimal, never use a zero after the decimal and include indications whenever possible. Miscalculations are preventable if proper methods of inspecting calculations are used.† (Preston  2004, p.72) Assess for the effects of drugs to avoid harming a client. A client must be assessed before and after a drug is given. For instance, before giving an oral medication, assess whether the client can swallow or feels nauseated. An appropriate follow up should be done after a medication is administered. It is important to check if the client experienced the desired effect of the drug. Significant abnormal responses to drug should be reported to the physician. (Kozier & Erb 2004) To finalize this assignment, medication errors are mistakes that can cause harm to patients and can even result in death. The articles that have been summarized illustrate situations where medication errors have occurred and review what the health care industry is doing to prevent errors. A medication error is preventable and errors can be caused by a variety of reasons. This paper has discussed the impact medication errors have on client care and strategies of how to prevent errors from occurring. Even the governments cannot respect our privacy. They put security cameras on every corner, in every building. Understandably they prevent and solve crimes but as a result it removes our privacy. We should respect their privacy as long as it does not affect how they act publicly. In conclusion, even though the media may not respect some individuals privacy and the governments may take ours as a bi-product of protecting us people should make more of an effort to keep some factors of their life private. As I said at the beginning people should retain some mystery and express themselves truly by what they do and how they do it. Yet, they may feel free to express themselves by telling us about their breakfast if it is a humorous anecdote or an interesting metaphor.

The Kittyhawk managers carefully selected their staff of exceptional employees from within HP, composed of risk-takers that would be more excited by the market potential of a 1. 3-inch drive than by its technological capabilities. HP has never been a pioneer in the disk-drive business, and Kittyhawk was considered a pioneering effort by the company. HP concentrated its efforts on the most productive stages of the NPD process. The core team for the development of the 1. -inch drive came from within the company. The whole process of development was done within the company but the project team outsourced the manufacture of the drive to an external supplier with proven expertise in miniaturized manufacturing Japans Citizen Watch Corporation which designed and built an automated production line for Kittyhawk. Initial market research was undertaken by S eymour and White but to give Spenner reassurance, the team contracted a highly reputable research firm that specialized in high tech markets to independently gauge the magnitude of Kittyhawks opportunity. However, as no clear market was evident yet, the research firm ended up deriving their conclusions from the Kittyhawk team. As a result, the results of the independent study only mirrored the thoughts of the team. This may have contributed to the error in the target market of the Kittyhawk. HP provided adequate resources and focused R;D funding to the project team which were critical to the success of its NPD process. Eventually, the Kittyhawk project failed to meet its goals but HP still acknowledged its value.

Restoration of 5-HT depletion impaired memory by Vortioxetine was also demonstrated by Jardin KG and Jansen JB through PCPA induced 5-HT depletion model and behavioural tests including object recognition (OR) and Y-maze spontaneous alternation (SA) tests respectively. SERT (Serotonin transporters) occupancy (>90%) by vortioxetine was established in their preclinical work, with reports of dose-dependently reversed recognition memory and spatial working memory in rats. Effectiveness in MDD relapse prevention (Long term therapy) Major depressive disorder (MDD) patients have to maintain therapy even after remission by acute treatment to prevent relapse especially when residual symptom and comorbid illnesses (E.g: MDD with generalized anxiety disorder) are present at the remission point (Keller M, 2007). The long term efficacy of antidepressants is also be established by study of its role in maintenance treatment in preventing depression recurrence in patients who responded to acute therapy (Kornstein 2006)(Steinert C 2014). Alam M et al (NCT 00707980) carried out an open-label, 52-weeks, long term study to identify efficacy of vortioxetine along with monitoring of adverse effects. This was the extension of two double blind, placebo controlled short term (8weeks) lead in studies involving participants suffering of MDD. Total 834 patients were treated with 5 mg/day dose of vortioxetine for first week followed by dose titration from 2.5 mg/day to 10 mg/day depending on intensity of response till 52 weeks. The study concluded with absence of potential adverse effect reports and proved efficacy of vortioxetine by maintenance of remission observed by Change From Baseline score of MADRS Total Score (Observation: -7.4  ±9.81 units Change from baseline score as end point result) and 24-item Hamilton Depression total score (Observation: -7.9  ±9.66 units change from baseline score as end point result). Furthermore, Baldwin DS et al reported safety and efficacy of vortioxetine in prevention of relapse of MDD o n the basis of 52 weeks of remission maintenance therapy in patients who previously completed acute therapy of vortioxetine for 8 weeks with evidence of remission. Patients reported approximately 8 points reduction from 13.5 ±8.7 base line MADRS score after 52 weeks of treatment indicated favorable profile of vortioxetine (2.5, 5 and 10 mg/day) in relapse prevention during long term remission maintenance therapy of MDD. At the end of 52 week treatment, approximately 2% patients have shown sign of relapse (MDRS score ≠¥ 22) as per Montgomery-Asberg Depression Rating Scale. For vortioxetine, a MDD relapse prevention study was carried out with participation of 639 MDD patients aged between 18-75 years and diagnosed with a current major depressive episode (MDE) within four weeks duration. In this investigation, against baseline score in MADRS scale ≠¥26, a remission point was considered as MADRS total score ≠¤10 for vortioxetine (5 or 10 mg/day) treated patients to enter in double-blind, 24 weeks phase of treatment with either placebo (n=192) or vortioxetine (n=204) for relapse time analysis. At the end of the study, relapse rate was 13% and 26% for vortioxetine and placebo treated groups respectively as an evidence of efficacy of vortioxetine in prevention of relapse in 396 patients participated in 24 week phase (Boulenger JP 2012). COMPARATIVE EFFICIENCY Treatment with vortioxetine (2.5 mg, 5 mg, 10 mg) failed to make differentiation from placebo when Change From Baseline in MADRS Total Score was considered as a primary efficacy analysis tool in a 8 Week randomized, double blind clinical study. In the same investigation, Duloxetine (60 mg), a reference drug used to find efficacy of vortioxetine, also failed to make differentiation against placebo in primary efficacy analysis. Secondary efficacy outcome analysis and tolerability analysis suggested efficacy of vortioxetine with 5mg and 10 mg strength through MMRM (Mixed-Effect Model Repeated Measure) model (Badwin DS, 2012). In a randomized, double blind study involving comparison of vortioxetine (n= 252) and agomelatine (n=241), superiority of treatment with vortioxetine (p Katona C et al reported significantly (P = 0.0011) higher efficacy of vortioxetine (5 mg/day) in comparison of placebo by 24-item Hamilton Depression Scale (HAM-D (24)) total score () as the primary measure. 29.2 vs. 19.3% remission rate were observed at the endpoint for vortioxetine and placebo controlled population respectively during the study which also involved duloxetine as active reference. The patient adherence rate was 94.2% in case of vortioxetine and 90.1% in case of duloxetine demonstrated tolerability of vortioxetine in their study. Comparative Safety of vortioxetine (10mg per day) in MDD treatment was established on the basis of 15 days study demonstrating its effect on psychomotor performance in 24 healthy subjects. The drug didn’t show alteration in psychomotor functioning withing and at the end point of te study. The investigation also involved use of Mirtazapine as an active reference which was found to be inferior through strandardized driving test due to cognitive impairment obsrved by it. (Theunissen). EFFECTIVENESS ON ETHNIC POPULATION Majority of the clinical investigations listed in table 3 were carried out at multi sites involving global population and diverse demographic distribution. The drug was found to be equally effective on participants from different countries. Recently H.Lundbeck announced head-to-head study results comparing efficacy and tolerability of vortioxetine 10mg/day and venlafaxine XR 150mg/day in treatment of MDD in 437 adult participants (Inclusion criteria: MADRS total score ≠¥26) from four Asian countries, Thailand, South Korea, Taiwan and China. The end point outcome established slightly more effectiveness of vortioxetine compared to venlafaxine with mean difference of -1.20 points in change observed in baseline MADRS total score(with 95% CI: -3.03 to -0.63). The superiority of vortioxetine was strongly supported by its tolerability data observed as lesser incidences of adverse events (6.6% vs. 13.7%) and lesser drop out ratio of participants (18% vs. 27.4%) in case of vortioxetine treatment compared to active reference. Efficacy of vortioxetine in Asian population was established by substantial improvement in MDD as evident by change in baseline score i.e.: -19.4 points on MADRS total score. SAFETY AND TOLERABILITY Efficacy and safety for Sexual function maintenance Sexual dysfunction is one of the major risks of antidepressant therapy particularly with selective serotonin reuptake inhibitors (SSRIs)(Strohmaier J 2011). Therapy discontinuation is reported frequently in clinical studies involving various antidepressants due to adverse effects associated with sexual problems which includes including anorgasmia, problems in ejaculation, absence of libido and impotence but still sexual dysfunction is one of the most under-rated AEs (Serretti A). at the same time symptoms and illness of depression is also known to be associated with sexual dysfunctions(Baldwin D, 2013 AND Foong T; Reichenpfader U,). In this dilemma, the data generated through efficacy and safety studies of vortioxetine are supporting the selection of vortioxetine as an antidepressant with the minimum side effects as far as sexual health impairment is concerned. A study involving MDD patients suffering of SSRI monotherapy (Treatment with citalopram, paroxetine or sertraline) induced sexual dysfunction indicated effectiveness of vortioxetine in clinically meaningful improvement in sexual functioning compared to escitalopram. This multicentre, randomized, double-blind, active controlled clinical trial was carried out on SSRI treated 447 participants who were already in partial or full remission state from MDD as measured by the MADRS and CGI-S/I(Clinical global impression severity and improvement scales). The participants were switched 10 mg/day fixed dose of vortioxetine or escitalopram for the first week of study. Further, the dose was increased to 20 mg/day in second week followed by flexible dose (10-20 mg/day) till the end of 8th week. The dose was taped down to 10 mg/day for escitalopram control group whereas vortioxetine treated group was switched to placebo control for another 1 week. Treatment with vortioxetine(n=169) concluded in a s tatistically significant treatment effect by change from baseline CSFQ-14(Changes in sexual functioning Questionnaire) total score at the end of 8th week of study in comparison of escitalopram(n=179). A mean treatment difference of 2.2 points for vortioxetine vs escitalopram groups(95% CI: 0.48—4.02; p=0.013, MMRM) proved efficacy of vortioxetine in sexual function restoration and MDD remission mintenance in patients treated with other antidepressant previously. Previously, during the proof-of-concept, short term efficacy study for vortioxetine (5mg/day and 10 mg/day) with placebo control and active reference control has demonstrated statistically higher safety of vortioxetine in comparison of active reference (12.4% incidences for venlafaxine vs. 1.9% incidences for 5 mg/day vortioxetine and placebo, p=0.0033, Fishers exact test) and equivalent safety in comparison to placebo group for the incidences of sexual dysfunction related adverse effects. In contrast, Mahbaleshwarkar 2013 (NCT00672620) reported high incidences (51% for and 37.5% for vortioxetine 2.5 mg and 5 mg versus 26.9% duloxetine and 33.3% placebo group) of treatment-emergent sexual function impairment by treatment with vortioxetine. They monitored sexual dysfunction as a adverse effect using the ASEX (Arizona sexual experience) scale. The study involving a total 9f 611 participants also concluded with observation of non-significant difference in changes from baseline of HAM-D2 4 total score between 5mg, 10 mg vortioxetine and placebo. The above studies indicate that sexual function related AEs is not a concern for long term use of vortioxetine as a remission maintenance therapy unlike other antidepressants whereas patients may suffer by adverse effects related with sexual dysfunction when the therapy for remission with 5mg or 10 mg dose is started in patients suffering of MDD. Adverse Effects Frequency and severity of adverse effect and change in selected vital body parameters, ECG, weight change due to vortioxetine (2.5, 5.0 and 10.0mg/day) administration were observed during a 52 week open labeled study by during a open label long term study by Baldwin DS. They also considered physical examination and clinical safety laboratory tests to make thorough investigation. Out of 535 participants, only 2% of individuals have shown sign of severe adverse effects but none of the adverse effect was prevalent in more than 0.56% individuals among total participants. Apart from this, majority of the patients reported the non-serious adverse effects have shown sign of nausea (19.81%) and headache (15.33%). Sexual dysfunction related adverse effects were reported in six patients at the end of the study. This study was also concluded with effectiveness of vortioxetine in long term treatment as mentioned previously in this article. The study was concluded with positive outcome in terms o f tolerability of vortioxetine for long term maintenance therapy in patients with MDD remission. Drug Interactions Vortioxetine dose adjustment is not required based on age, race, gender, ethnicity, renal or hepatic impairment but due to serotonin syndrome and neuroleptic malignant syndrome, the drug is contraindicated in concomitant administration with irreversible non selective MAOIs(Monoamino oxidase inhibitors). Chen G et al reported drug interaction cases affecting pharmacokinetic profile of vortioxetine in healthy subjects. They observed need of dosage adjustment of vortioxetin when it is concomminately administered with bupropion, rifampicin as steady state AUC and Cmax of of vorteoxetine enhanced by 128 and 114% in case of bupropion co-administration and suppressed by 72% and 51% in case of rifampicin co-administration. At the same time, tolerability of simultaneous therapy was evident by absence of any severe adverse effects. They also reported safety of vorteoxetine in concomminent administration of popular contraceptive, Estradiol. Vortioxetine is mainly metabolized in liver by CYP2D6 and up to some extent by CYP2C9 and CYP34/5. This makes vortioxetine sensitive to other drugs which are CYP2D6 inhibitors, CYP3A4 and CYP2C9 inhibitors and Cytochrome P450 inducers. Other than these, reversible and non-selective MAOI and serotonergic medicinal products are also potential drug substances affecting efficacy and safety of vortioxetine.

Fluctuating Gas Prices Will Never End Essay -- essays research papers

These days, gas prices seem to be a weekly topic on local and national news channels. Every time there is some type of fluctuation, it causes an uproar, usually because it is a difference of ten cents or greater. The reasons as to why is definitely a mystery with new myths popping up each day and almost all the supposed reasons being incorrect. The real discussion should be not as to what causes the price change but how it affects our everyday lives. Should we adjust our way of life to accommodate less driving or have we already? In the U.S., we saw gas prices go as low as $1.75 and as high as $3.00 in 2005. In 2006, we saw almost the same trend with a low of $2.10 and so far we have leveled out around $3.00 or so. While I can agree that $1.75 works a lot better on my budget with two kids and a lot of ground to cover in my SUV each day, there is actually some good reason as to why we see such change each year. The first and most obvious reason would have to be that during the winter months when we drive the least, prices drop to accommodate the resulting increase in supply and the opposite occurs in summer months when we drive the most. Another major cause in price change occurs when there is a change in value in crude oil, also dependant on the supply and demand factor. With many large worldwide issues such as weather and war affecting the supply, this can be part of the reason but is definitely not the main factor as many people believe. You may be pondering though, â€Å"Why does the price of fuel go up with the price of each barrel but not back down with it as well?† That is a simple answer as well – supply and demand. With the prices rising, less gas is purchased during a time that the companies have to sell extra to make ... ...ust get in the car and drive for no reason? I definitely do not, I don’t have time too! And I’m sure most other people don’t either. When we get in our cars, we usually have somewhere to be and I need groceries and I need to go to work so I can’t cut down on how much I drive even if I wanted to. So in conclusion, we should all quit blaming the politicians, who in fact have lowered the taxes by a small margin to assist during the hard times and the oil companies who have faced the largest ratio of production costs in history. So unless you can setup a carpool and split the cost of gas with others or are willing to utilize public transportation, there is no sense in worrying about what you can do because there isn’t much we can do. We should adjust our budgets and not our way of life because an increase in everyday life costs is inevitable and just a fact of life.

Am I Blue?

English 9-2 7 May 2012 Judged The saying, â€Å"do not judge a book by its cover† is a commonly used saying throughout the years. However, what does the saying really mean? Of course an individual should not be choosing the novel they wish to read just based on the cover of the book. Figuratively speaking, the saying means that one should not prejudge the value of someone just by their physical appearance alone. For many people, the root of their anxiety stems from the fear of others judging or evaluating them because of they way they look or act.The short stories â€Å"Am I Blue? † by Bruce Coville and â€Å"Hum† by Naomi Shihab Nye both illustrate the uselessness of judging a book by its cover. In the short story â€Å"Am I Blue? † by Bruce Coville, Vince, the main character, discovers that people are not always what them seem. In this story Vince, who is questioning his sexuality, is the main target for harassment because his classmates think he is gay. When Vince receives his very own fairy godfather, he makes a wish that turns everyone who is gay the color blue.Vince is surprised when he sees that many people turn even the slightest shade of blue. The darker the shade, the more gay they are. Vince’s fairy godfather tells him, â€Å"my friends and I called each other ‘faggot’ and ‘queer’ for the same reason so many black folks call each other ‘nigger’ –to take the words away from the people who want to use them to hurt us† (Coville 10). People who are gay often say words such as, â€Å"faggot† and â€Å"queer† to mask themselves or make those words less hurtful to them.As Vince walks around town, he sees all sorts of people who are all different shades of blue. Vince says that â€Å"it was like seeing the world though new eyes. Most of the people looked just the same as always, of course. But Mr. Alwain, the fat guy at the grocery store, looked like a gian t blueberry—which surprised me, because he was married and had three kids† (Coville 12). When Vince returns home and turns on the television, he is enraged when he sees â€Å"the congressman [on TV was the shade of a spring sky], who happened to be a notorious republican homophobe† (Coville 13).Vince even discovers that the bully picking on him for being gay is also gay himself. People fear others judging or evaluating them because of they way they look or act, so they often put on a facade. How someone acts or looks is not an accurate reflection of who they truly are. In the short story â€Å"Hum† by Naomi Shihab Nye, Sami, the main character discovers what it is like to have others judge him because of the way he looks. Sami and his family move to America from Palestine a few weeks before the 9/11 terrorist attack on the World Trade Center.Sami and his family think their lives are improving. Sami is making friends and adjusting to his new life in America . When 9/11 occurs, it shocks Sami and his family. Sami says that â€Å"his family had always spoken out against the suicide bombings that killed Israeli civilians [in Palestine]. Many Palestinians did. But who could hear them? They were regular people, not politicians. No one quoted them in the news† (Nye 74). Sami knows his people are not murderers. No one says the hijackers are Palestinian.Yet because Sami looked so similar to the terrorists in the attack, everyone blames him and his people for what happened. When he goes to school, â€Å"No one sat with him at lunch now. He tried sitting down next to some boys from his PE class and they stopped speaking and stared at him. ‘I feel very bad about what happened’ Sami said. [†¦] Nobody answered him. They finished eating in silence, exchanging glances with one another, and left the table† (Nye 75). When Sami meets a blind man who is his neighbor, the man tells him that â€Å"some people don’t u se [their inner eyes] enough.They forget about them. But there’re all I have. In some ways, I think I can see better than people who aren’t blind† (Nye 83). Because the man is blind, Sami can become friends with his neighbor without his looks getting in the way.
